ABSTRACT This case study details the case of a 19-year-old female who was presented to the emergency department with a rapid progression of dyspnea from grade III to grade IV over 48 hours. The patient's medical history included a two-day fever, a week-long cough with brownish expectoration, and recent episodes of vomiting. Notably, she had been asymptomatic until a week prior when she experienced a low-grade fever that responded temporarily to medication, followed by the development of a high-grade fever with rigors and chills, lacking myalgia or arthralgia. The cough evolved to foul-smelling with bloody streaks, and dyspnea escalated from grade 3 to grade 4, exacerbated by orthopnea. The absence of paroxysmal nocturnal dyspnea (PND) and no reported allergies were observed, while the patient had a known history of dust allergy and seasonal allergic reactions, denying inhaler use.
