Study On the Indications of Inborn Nursery Admission and The Factors Associated With Neonatal Death

ABSTRACT
Introduction
Newborn health is the key to child health and survival. It is an Important phase in life. Almost half of under five deaths occurs in newborn period most of the causes are preventable and manageable.In-Born Nursery (IBN) aims at giving care for babies who are born early, who develops problems during delivery.But IBN provides benefits as well as challenges. A baby admitted in IBN suffers from exposure to continual light, high level of noise, interference with breast feeding & reduced body contact due to separation from mother. So we decided to study on indications and outcome of IBN admission, to suggest measures that can be adopted prenatally & postnatally to reduce such admission.
OBJECTIVE
To study about the indications of In-Born Nursery admissionand the factors associated with neonatal death in In-Born Nursery of a tertiary care center.
METHODOLOGY
A hospital based cross sectional study was done.150 neonates admitted to Inborn nursery of a tertiary care centerin a period of two months were taken as study subjects. The study was done using hospital records and semi structured questionnaire. Sampling technique used was convenient sampling.
RESULT
On analysis it was found that 53.3 % of babies admitted suffered from Respiratory distress soon after birth(RDSAB), 38.7% were LowBirthWeight, 38% were Preterm. There was a strong association of neonatal death with gestational age (adjusted OR=6.522), APGAR at 5’ (adjusted OR = 5.3306), duration of IBN admission (adjusted OR = 2.927). Maternal pregnancy induced hypertension (PIH) was found to have a protective effect (adjusted OR = 0.205).
CONCLUSION AND RELEVANCE
Low Birth Weight, Prematurity, Birth asphyxia were found to be the major reasons for IBN admission. Prematurity and low APGAR score at 5 minutes (<7) are found to have association with neonatal death.
Close monitoring of vitals and timely management can reduce their mortality. Labour rooms must have adequate facilities and staffs with good clinical acumen to tackle birthasphyxia in order to reduce neonatal inborn admission and neonatal morbidity and mortality .
